About The Position

The LPN provides nursing care to a select group of residents in an assisted living and memory care unit. Under the supervision of a SALSA, the nurse provides supportive and restorative care, teaches residents and families, and collaborates in implementing resident services plans. The position is based at Masonicare at Mystic in Mystic, Connecticut, and is scheduled for the evening shift, 32 hours per week, including every other weekend.

Requirements

Graduate of an accredited School of Nursing.
Current LPN license.
CPR certification.

Nice To Haves

1–2 years of clinical experience in a hospital or similar health care setting is preferred.

Responsibilities

Contribute to resident care and service plans by collecting data based on residents’ ages, needs, and abilities, and participating in regular care-planning meetings.
Communicate and collaborate with residents, families, colleagues, and other health care team members to meet resident needs.
Educate, supervise, and counsel residents and families regarding assisted living procedures and other care needs.
Document accurate, pertinent, and comprehensive information in the client service record in a timely and concise manner.
Conduct supervisory visits of resident care assistants, companions, and homemakers as required.
Participate in educational programs, in-services, meetings, and workshops, and use current literature to enhance professional skills and knowledge.
Under the supervision of the SALSA/RN designee and in collaboration with physicians, pharmacists, and outside agencies, oversee medication management, including ordering, storing, reconciling, and preparing medications for administration and/or CNA prompt.
Perform other duties as assigned.
