LPN (Licensed Practical Nurse) Home Health
About The Position
Phoenix Home Health is hiring a full-time LPN to provide home health nursing care in Branson, Missouri, and surrounding areas. The LPN serves as a link between patients and medical providers, carrying out individualized care plans based on physician orders and initial assessment findings. The role involves collaboration with nurses, physical therapists, speech therapists, and occupational therapists. Charting is completed on-site with assistance from Roger’s AI-powered support. The schedule is Monday through Friday, 8 a.m. to 5 p.m., with a weekend rotation every four to six weeks. The listed pay range is $50,000–$80,000.
Requirements
- Missouri or Compact Registered Nursing or Licensed Practical Nursing license.
- Strong organizational and self-management skills.
- Strong and compassionate customer service skills.
- Valid driver’s license.
Responsibilities
- Assist in reviewing and revising the plan of care.
- Provide nursing care in accordance with the physician’s plan of treatment and the individual plan of care, as authorized by the client’s payer source.
- Observe and record the client’s reaction to treatment and report changes in the client’s condition to appropriate personnel and/or the physician.
- Provide ongoing instruction to the client or family regarding the plan of treatment.
- Participate in education programs, quality assessment activities, nurses’ meetings, and other staff meetings as required.
