Licensed Practical Nurse (LPN) Home Health
About The Position
Phoenix Home Health is seeking a Licensed Practical Nurse to provide home health nursing care in Stockton, Missouri, and surrounding areas. The LPN serves as a link between patients and medical providers, using physical assessment and communication skills to carry out individualized care plans based on physician orders and initial assessment findings. The role involves collaboration with nurses, Physical Therapists, Speech Therapists, and Occupational Therapists. Nurses complete charting on-site with assistance from Roger’s AI-powered support, rather than taking work home. The schedule is Monday through Friday, 8 a.m. to 5 p.m., with a weekend rotation every four to six weeks. Requirements
- Missouri or Compact Registered Nursing or Licensed Practical Nursing license.
- Strong organizational and self-management skills.
- Strong and compassionate customer service skills.
- Valid driver’s license.
Responsibilities
- Assist in reviewing and revising the plan of care.
- Provide nursing care in accordance with the physician’s plan of treatment and the individual plan of care, as authorized by the client’s payer source.
- Observe and record the client’s reactions to treatment and report changes in the client’s condition to appropriate personnel and/or the physician.
- Provide ongoing instruction to the client or family regarding the plan of treatment.
- Participate in education programs, quality assessment activities, nurses’ meetings, and other staff meetings as required.
