About The Position
Castor Home Health is seeking a full-time LPN Intake Coordinator to manage home health referrals from receipt through admission. This position works closely with patients, families, referral sources, clinical leadership, and scheduling to support timely and well-coordinated starts of care.
Requirements
- Current, unrestricted Illinois LPN license.
- Strong communication, organization, and follow-through.
- Ability to manage multiple priorities in a fast-paced environment.
Nice To Haves
- Healthcare intake, home health, care coordination, or related experience preferred.
- Knowledge of home health referral and admission processes is a plus.
- Experience with an electronic medical record system is preferred.
Responsibilities
- Receive and review new home health referrals.
- Contact patients and families promptly to confirm interest and readiness for services.
- Review referral documents and obtain missing orders or clinical records.
- Coordinate insurance verification and authorization requirements.
- Communicate with physicians, hospitals, facilities, and other referral sources.
- Work with clinical leadership to address eligibility or clinical questions.
- Coordinate accepted referrals with the scheduling and field teams.
- Track pending referrals, follow up on delays, and escalate barriers.
- Maintain accurate referral statuses and documentation.
- Support timely admissions and a positive patient experience.
