About The Position

Care IV Home Health is hiring an LPN Intake Coordinator for a full-time, office-based role at its corporate headquarters in Little Rock, Arkansas. This is a non-patient-care position that supports the Intake Department and reports to the Intake Manager. The role requires strong attention to detail, clinical nursing knowledge, and excellent computer and communication skills. Some rotating on-call work may eventually be required.

Requirements

Active Licensed Practical Nurse license in good standing with the Arkansas State Board of Nursing.
Recent experience working in a clinic or other professional medical office setting.
Excellent computer skills and working knowledge of office machinery, including computers, multi-line phones, fax machines, and email.
Solid clinical nursing knowledge and previous skilled nursing experience.

Nice To Haves

Home Health experience is preferred but not required.

Responsibilities

Take calls from prospective home health patients and referral sources.
Describe Care IV's home health service options to referral sources and patients.
Evaluate patients over the phone and determine whether they are appropriate for home health care.
Verify that patients' payer sources are in network with Care IV.
Assist the Intake Manager with orders and other clinical information.
Send and receive faxes containing supporting documentation, signed orders, and related materials.
Follow up with physicians, clinics, long-term care facilities, hospitals, and other contacts.
Coordinate new patient onboarding with the Marketing Team to support satisfactory customer service for referral sources and patients.
Enter new patient data into the computer system and assist the RN with tracking and data reporting.
Perform additional duties as assigned.
