About The Position

We are actively searching for an LPN Hospice nurse to join the team. The nurse provides skilled, compassionate care to hospice patients, supports symptom management, monitors changes in condition, educates patients and families, and works closely with the interdisciplinary care team. The role provides direct nursing care in home and facility settings.

Requirements

• Current LPN license valid for practice in Missouri.
• Current driver's license.
• Minimum of one year of nursing experience.
• Hospice or palliative care experience.
• Reliable transportation and valid automobile liability insurance.

Responsibilities

• Provide direct nursing care to hospice patients in home and facility settings.
• Assess patient needs and monitor symptoms, vital signs, and changes in condition.
• Administer medications, treatments, and interventions as ordered.
• Educate and support patients and families regarding symptom management and end-of-life care.
• Communicate changes and concerns promptly to the RN and interdisciplinary care team.
• Complete all required documentation accurately and on time.
