About The Position

Franciscan Health is seeking a Licensed Practical Nurse to provide home-based patient care. The LPN implements the nursing plan of care under the direction of a registered nurse, assesses patient needs, provides appropriate interventions, and revises care as needed under RN supervision. The role also includes educating caregivers and participating as a member of the patient’s care team. The LPN collaborates with the Home Health RN and multidisciplinary treatment team to individualize care and work toward defined patient outcomes. The position involves assessing patients’ psychosocial needs, contributing to discharge planning, identifying post-discharge needs and follow-up resources, and maintaining timely and accurate patient care records. Travel is required up to 75%.

Requirements

Associate’s degree or diploma from an accredited LPN program.
At least 1 year of acute care, long-term care, or home health experience.
Licensed Practical Nurse (LPN) credential from the state licensing board.
Cardiopulmonary Resuscitation (CPR) certification from the American Heart Association.
Driver’s license.
Auto insurance.

Responsibilities

Implement the nursing plan of care at the direction of the registered nurse.
Provide home-based patient care according to physician orders and the established nursing plan of care.
Collaborate with the Home Health RN and multidisciplinary treatment team to individualize treatment and achieve defined patient outcomes.
Conduct psychosocial assessments addressing patients’ physical, psychological, and social circumstances; obtain histories and identify psychosocial needs.
Contribute to discharge plans by identifying post-discharge needs and appropriate follow-up resources in collaboration with patients, families, and the care team.
Document patient care and complete patient care records accurately and on time in accordance with policies and procedures.
Educate caregivers and participate in the patient’s care team.
