About The Position

Lynn Haven Health & Rehab is seeking an LPN Charge Nurse to deliver high-quality clinical care and support residents’ well-being. The nurse works closely with RNs, CNAs, and the interdisciplinary team to provide compassionate, person-centered care in a non-profit, mission-driven organization. The role includes supervising and supporting CNAs and communicating with residents, families, and team members.

Requirements

• Active Licensed Practical Nurse (LPN) license in the State of Georgia or eligibility to obtain one.
• Passion for delivering compassionate, high-quality care.
• Strong clinical judgment and critical-thinking skills.

• Reliability and commitment to excellence.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents’ conditions and report changes to the care team.
• Provide direct nursing care in accordance with care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with clinical policies and regulatory standards.
• Communicate effectively with residents, families, and team members.
