About The Position
Four County Health & Rehab is hiring a PRN Licensed Practical Nurse Charge Nurse to provide compassionate, person-centered clinical care. The nurse works closely with RNs, CNAs, and the interdisciplinary team to support residents’ well-being and ensure high-quality care. The role includes supervising and supporting CNAs, communicating with residents and families, and following clinical policies and regulatory standards. Prior long-term care experience is preferred but not required.
Requirements
- Active Licensed Practical Nurse (LPN) license in the State of Georgia, or eligibility for one.
- Passion for delivering compassionate, high-quality care.
- Strong clinical judgment and critical thinking skills.
- Excellent communication and teamwork abilities.
- Reliability and a commitment to excellence.
Nice To Haves
- Prior long-term care experience.
Responsibilities
- Administer medications and treatments as prescribed.
- Monitor residents’ conditions and report changes to the care team.
- Provide direct nursing care in accordance with care plans.
- Supervise and support CNAs in delivering quality care.
- Maintain accurate and timely documentation.
- Ensure compliance with clinical policies and regulatory standards.
- Communicate effectively with residents, families, and team members.
