LPN Acute Care - Complex Discharge Unit

Legacy Health•Portland, OR
•$30 - $43•Onsite

About The Position

The Licensed Practical Nurse (LPN) provides patient care within an assigned acute care nursing unit or department as a member of the health care team, in cooperation with and under the direction of a registered nurse. The role contributes to nursing assessments and diagnoses, participates in developing and evaluating care plans, and provides care according to established nursing and unit standards, including standards for patient assessment and transfer. The Complex Discharge Unit cares for medically stable patients with complex needs who are awaiting a safe discharge plan. Patients may need ongoing treatment, such as wound care or IV antibiotics, and may remain in the hospital when that care cannot safely be completed elsewhere. LPNs may work alongside acute care RNs, though not necessarily in direct acute patient care areas, and collaborate with CNAs, charge RNs, unit coordinators, physicians, patients, and families. Critical thinking, problem-solving, willingness to learn, clear communication, compassion, and teamwork are important to the role. The unit supports staff learning and continued education.
