LPN Acute Care - Complex Discharge Unit

Legacy Health•Portland, OR

About The Position

The Licensed Practical Nurse (LPN) works in an acute care nursing unit serving medically stable patients with complex needs who are awaiting a safe discharge plan. Many patients require ongoing treatment that cannot yet be completed elsewhere. The role may provide hands-on experience with wound care, IV antibiotics, and other longer-term treatments. LPNs may work alongside acute care RNs, though not directly in acute patient care areas, and collaborate with CNAs, charge RNs, unit coordinators, and physicians. Under the direction of a registered nurse and in cooperation with the health care team, the LPN provides patient care within an assigned acute care unit or department. The LPN contributes to nursing assessment and the establishment of nursing diagnoses, participates in care-plan development and evaluation, and provides care according to established nursing and unit or department standards, including standards for patient assessment and transfer. Critical thinking, problem-solving, communication, compassion, teamwork, and willingness to learn are important to this role. The team supports learning and continued education for staff.
