About The Position

The LPHA Mobile Crisis Outreach Team (MCOT) team member works under the direction of the Supervisor of MCOT and provides team-based, face-to-face crisis assessments, crisis intervention, crisis follow-up, and relapse prevention services for individuals with chronic mental illness in Collin County. Services are provided to children, adolescents, and adults physically located in Collin County at the time of crisis, at residences, schools, hospitals, jails, and other safe community-based locations. The program operates 24 hours per day, 365 days per year, including holidays. The team member coordinates and monitors services, conducts crisis assessments and safety planning, advocates and liaises on behalf of individuals served, and provides follow-up for up to 90 days. Within that period, the team member also provides routine case management, skills training, psychosocial rehabilitation, and community resource education in accordance with federal, state, and local guidelines. Services are based on a trauma-informed and person-driven care model. The position involves initiating and collaboratively completing uniform assessments, organizing and documenting recovery plans to address identified needs, and discharge planning. The team member may transport individuals in a center vehicle. This position is cross-trained with the Crisis Center to assess crisis resolution and stabilization and link individuals to services with the goal of reducing inpatient and law-enforcement interventions. Additional duties may be assigned to meet program and individual needs. Expectations include timely and thorough documentation, effective time management and organization, active collaboration with community partners, and strong teamwork. Specific targets include completing assessments as soon as possible and within the response time required by each case’s urgency; completing and signing documentation concurrently by the end of the shift; completing assigned follow-ups at 24 hours, days 1–7, and 30-, 60-, and 90-day intervals; communicating a shift report at the end of each shift; achieving a score of 80 or higher on 90% of full chart reviews; and attending monthly team meetings. The position requires working two 12-hour shifts on Saturday and Sunday. Because MCOT operates 24/7, employees may be required to work center holidays when a holiday falls on their scheduled workday. When unforeseen circumstances require use of Paid Time Off, employees are expected to notify their supervisor within four hours of their regularly scheduled work time. Bilingual employees receiving a stipend must be available to translate for other staff as needed and use approved language in service provision as needed. The listed compensation is $35.58 per hour, with a $4,000 LPC recruitment and retention bonus paid as $1,000 after 90 days, $1,000 after six months, and $2,000 after 12 months.
