About The Position
MedCap Health is seeking a Long Term Care Doctor – Post-Acute Care Physician to provide care in skilled nursing facilities. The physician will lead care teams and manage complex cases of patients discharged early from hospitals, collaborating with Nurse Practitioners and specialists to deliver hospital-level care in a long-term setting. The role supports continuity, quality, and compassionate patient care. The position offers flexible scheduling, including full-time, part-time, or customized hours.
Requirements
- Board certification or board eligibility in internal medicine, critical care, family practice, or geriatrics.
Nice To Haves
- Previous experience in leadership positions or as a hospitalist is strongly preferred.
