About The Position
MedCap Health is seeking a Long Term Care Doctor – Post-Acute Care Physician to provide care in skilled nursing facilities. The physician leads care teams managing complex cases of patients discharged early from hospitals and collaborates with Nurse Practitioners and specialists to deliver hospital-level care in a long-term setting. The role emphasizes continuity, quality, and compassionate patient care, with schedule flexibility that may accommodate full-time, part-time, or customized hours.
Requirements
- Board certified or board eligible in internal medicine, critical care, family practice, or geriatrics.
Nice To Haves
- Previous experience in a leadership position or as a hospitalist is strongly preferred.
