About The Position
MedCap Health is seeking a Long Term Care Doctor – Post-Acute Care Physician to lead care teams in skilled nursing facilities and manage complex cases for patients discharged early from hospitals. The physician collaborates with Nurse Practitioners and specialists to deliver hospital-level care in a long-term setting, supporting continuity, quality, and compassionate patient care. The role offers full-time, part-time, or customized scheduling options.
Requirements
- Board certified or board eligible in internal medicine, critical care, family practice, or geriatrics.
Nice To Haves
- Previous experience in leadership positions or as a hospitalist is strongly preferred.
