About The Position
MedCap Health is seeking a Long Term Care Doctor – Post-Acute Care Physician to provide care in skilled nursing facilities. The physician will lead care teams and manage complex cases involving patients discharged early from hospitals, collaborating with Nurse Practitioners and specialists to deliver hospital-level care in a long-term setting. The role supports continuity, quality, and compassionate patient care. The schedule can be full-time, part-time, or customized to fit professional and personal goals. The position is described as a contract role and offers $100,000–$250,000+ per year. MedCap Health provides physician autonomy, schedule flexibility, and administrative support.
Requirements
- Board certified or board eligible in internal medicine, critical care, family practice, or geriatrics.
Nice To Haves
- Previous experience in leadership positions or as a hospitalist is strongly preferred.
