About The Position

FreedomCare is a healthcare company founded in 2016 that is revolutionizing home care delivery. We are expanding our operations to other states and are looking for a Part Time Licensed Social Worker (LSW) for our team in Ohio. This is a part-time, hybrid role that requires fieldwork and commuting to our Cleveland, OH office at least twice a month. The Licensed Social Worker will be integral to our Structured Family Caregiving (SFC) program in Ohio, which provides caregiving arrangements where a patient lives with a primary caregiver who offers daily support based on the patient's needs. The program aims to provide necessary care while promoting patient independence in a home environment, offering various care options as needs evolve. It is designed for individuals who require nursing facility-level care, prioritizing patient dignity, self-respect, and privacy. In this role, you will oversee patient care by supporting both the patient and caregiver, acting as an escalation point and advocating for quality service within FreedomCare and the SFC program.

Requirements

  • Valid and active Ohio Licensed Social Worker license, issued by the CSWMFT Board.
  • Bachelor’s degree (BSW) or Master’s degree (MSW) in Social Work from a CSWE-accredited program.
  • Minimum of 2 years clinical experience.
  • 1 year of experience with elders or adults with disabilities.
  • Valid driver’s license with an insured vehicle for travel within the state.
  • Demonstrated success driving operational excellence by effectively evaluating operational tasks as well as setting, achieving and measuring targeted outcomes, in a hybrid and field-based environment.
  • A high level of adaptability and experience in case management, with a fast-paced environment, presenting a high degree of autonomy, ambiguity, and accountability.
  • Excellent verbal and written communication, with the ability to effectively cascade information across diverse target demographics internally and externally.
  • Ability to represent FreedomCare holistically, with high emotional intelligence, empathy, and consciousness of end-user experiences (employees and customers).

Nice To Haves

  • Home care experience a plus.
  • Bilingual in Spanish is a plus.
  • Experience with CRM (Salesforce is a plus).

Responsibilities

  • Interpret the philosophy and objectives of the agency to clients, families, and others.
  • Ensure compliance with HIPAA, Ohio Department of Medicaid and Ohio Department of Aging rules and regulations, provider policies, FreedomCare policies/procedures, and state/federal mandates.
  • Conduct initial in-person home assessments with the patient and caregiver to ensure compliance and clarify the plan of care.
  • Provide day-to-day monitoring with monthly contact (telephonic and in-person visits, with no more than sixty days between in-person visits) with both patient and caregiver regarding patient enrollment, satisfaction with care delivery, and caregiver support.
  • Ensure program integrity and reporting related to SFC programs, and triage issues to appropriate internal staff.
  • Monitor quality of service within the required plan of care, providing individualized coaching, continued education, and reviewing caregiver goals and needs to enhance competencies.
  • Maintain detailed and accurate case records, documenting significant changes and observations in accordance with agency policy and procedure.
  • Act as the point of escalation and provide input to the client’s plan of care, including the physician’s plan of treatment and problem list, implementing and monitoring changes as necessary.
  • Perform other job-related duties as required or directed by the supervisor.
