Licensed Practical Nurse (LPN) Acute Rehabilitation
About The Position
The Licensed Practical Nurse at Baptist Health Rehabilitation Hospital delivers direct and indirect nursing care to patients during their acute rehabilitation and recovery. The role uses the nursing process to support patients’ health, independence, strength, and quality of life. The nurse collaborates with patients, families, and the interdisciplinary care team on individualized care plans, communicates changes in patient condition, and contributes to discharge planning for safe transitions. This is a full-time, day-shift position in an inpatient acute rehabilitation hospital.
Requirements
- Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing, or Nursing Diploma program.
- Current state licensure as a Licensed Practical Nurse.
- CPR/BCLS certification.
- Excellent oral and written communication and interpersonal skills.
Nice To Haves
- ACLS certification.
- At least six months of medical/surgical experience in an acute care setting.
Responsibilities
- Deliver direct and indirect patient care using the nursing process of assessment, planning, intervention, and evaluation.
- Collaborate with patients, families, and the interdisciplinary care team to create personalized care plans.
- Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues.
- Participate in discharge planning to support safe, successful transitions.
