About The Position

TGH Rehabilitation Hospital is seeking a Licensed Practical Nurse to provide acute rehabilitation care. In this PRN role with a varied schedule, the nurse supports patients throughout their recovery and helps them regain independence, strength, and quality of life. The position involves providing direct and indirect care, working with patients, families, and the interdisciplinary care team, monitoring patient conditions, and supporting discharge planning.

TGH Rehabilitation Hospital is an 80-bed inpatient acute rehabilitation hospital providing individualized care for people recovering from conditions including stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury. The hospital is accredited by The Joint Commission and CARF.

Requirements

• Graduate from an accredited Associate Degree in Nursing or Nursing Diploma program.
• Hold current state licensure as a Licensed Practical Nurse.
• Hold CPR/BCLS certification.
• Have excellent oral and written communication and interpersonal skills.

Nice To Haves

• ACLS certification is preferred.
• A minimum of six months of Medical/Surgical experience in an acute care setting is preferred.

Responsibilities

• Deliver direct and indirect patient care using the nursing process: assessment, planning, intervention, and evaluation.
• Collaborate with patients, families, and the interdisciplinary care team to create personalized care plans.
• Monitor changes in patient conditions and communicate them to physicians, supervisors, and colleagues.
• Participate in discharge planning to support safe and successful transitions.
