Licensed Practical Nurse (LPN) - Acute Rehabilitation - PRN
About The Position
TGH Rehabilitation Hospital is seeking a Licensed Practical Nurse to support patients during their acute rehabilitation and recovery. The nurse delivers direct and indirect care through the nursing process, works with patients, families, and the interdisciplinary care team to develop personalized care plans, monitors and communicates changes in patient conditions, and contributes to discharge planning to help ensure safe transitions. The role supports patients in building independence, strength, and quality of life. The hospital is an 80-bed inpatient acute rehabilitation facility providing individualized care for people recovering from stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury. Requirements
- Graduate from an accredited Associate Degree in Nursing or Nursing Diploma program.
- Hold current state licensure as a Licensed Practical Nurse.
- Hold CPR/BCLS certification.
- Have excellent oral and written communication and interpersonal skills.
Nice To Haves
- ACLS certification preferred.
- At least six months of medical/surgical experience in an acute care setting preferred.
Responsibilities
- Deliver direct and indirect patient care using the nursing process: assessment, planning, intervention, and evaluation.
- Collaborate with patients, families, and the interdisciplinary care team to create personalized care plans.
- Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues.
- Participate in discharge planning to support safe, successful transitions.
