About The Position

This is a temporary assignment for a Licensed Practical Nurse (LPN) in a 98-bed post-acute, long-term care/skilled nursing unit in Middlebury, VT. The contract duration is 4 weeks, with a start date of 08/13/2026. The shift is 3x12 Night shifts. The role requires floating to other units as needed and holiday coverage. Public transportation is not available in the area, so reliable transportation is required.

Requirements

  • Active LPN license on file at submission; appropriate licensure to practice in Vermont.
  • BLS certification required.
  • EPIC EMR experience within the past 2 years required.
  • Minimum 1 year of clinical experience.
  • Prior travel experience required.
  • Floating ability; willingness to work across units as needed.
  • Must be able to work all holidays; schedule includes holiday coverage.
  • Reliable transportation required; public transportation not available in the area.

Nice To Haves

  • IV certification preferred.
  • Dementia-capable care certification preferred.
  • 1+ year in a post-acute/skilled nursing rehab setting preferred.

Responsibilities

  • Provide direct nursing care to post-acute and long-term residents within scope of practice for an LPN.
  • Administer medications and therapies per physician orders; monitor for adverse reactions and document in the EPIC EMR.
  • Perform phlebotomy, EKGs, and assist with transports as needed; support nebulizer to inhaler treatments and starting IVs where certified.
  • Assess patient status, monitor vitals, wound care, and ADLs; report changes to the charge nurse and care team.
  • Collaborate with the Unit Leader/Director of Nursing and interdisciplinary team to optimize care plans; participate in rounds and care conferences.
  • Float to other units as required to support patient care needs; adhere to infection control and safety protocols.
