About The Position

The Licensed Practical Nurse (LPN) provides nursing care to a select group of residents in an assisted living and memory care unit. Under the supervision of a SALSA, the LPN provides supportive and restorative care, teaches residents and families, and collaborates on implementation of resident services plans. The position is per diem and is identified as a night-shift role at Masonicare at Mystic in Mystic, Connecticut.

Requirements

• Graduate of an accredited School of Nursing.
• Current LPN license.
• CPR certification/registration.

Nice To Haves

• 1–2 years of clinical experience in a hospital or similar health care setting is preferred.

Responsibilities

• Contribute to resident care and service plans by collecting data based on residents’ age, needs, and abilities, and participate in regular care-planning meetings.
• Communicate and collaborate with residents, families, colleagues, and other health care team members to meet resident needs.
• Educate, supervise, and counsel residents and families regarding assisted-living procedures and other care needs.
• Document accurate, pertinent, and comprehensive information in client service records in a timely and concise manner.
• Conduct supervisory visits of resident care assistants, companions, and homemakers as required.
• Participate in educational programs, in-services, meetings, and workshops, and use current literature to enhance professional skills and knowledge.
• Under the supervision of the SALSA/RN designee and in collaboration with physicians, pharmacists, and outside agencies, oversee medication management, including ordering, storing, reconciling, and preparing medications for administration and/or CNA prompting.
• Perform other duties as assigned.
