About The Position

This is a local, temporary assignment for a Licensed Practical Nurse in a Skilled Nursing Facility (long-term care) setting in Gallup, New Mexico. The contract is for seven weeks, with an option for extension. The role involves providing direct patient care, administering medications, monitoring residents, and collaborating with the interdisciplinary care team.

Requirements

• Active LPN license in the state of New Mexico in good standing (or eligible for NM licensure)
• Completion of an approved Practical Nursing program
• Prior experience in a long-term care or skilled nursing facility setting preferred
• Ability to work the listed shift hours (07:00–19:30) on a local assignment
• Strong communication and teamwork skills; proficient in basic documentation
• Must pass background check and drug screen as required and comply with facility policies

Responsibilities

• Administer prescribed medications and document according to policy and MAR
• Monitor residents, assess vital signs, and report changes to the supervising RN
• Provide direct patient care to support activities of daily living (ADLs), mobility, hygiene, and feeding
• Assist with wound care and dressing changes as ordered
• Collaborate with the interdisciplinary care team to implement and update care plans
• Maintain accurate documentation and participate in shift handoffs to ensure continuity of care
