About The Position

As a Licensed Practical Nurse (LPN) at Hickory Creek at Rochester, you will build meaningful relationships with residents and staff by learning about their life stories, routines, and preferences. You will use your nursing skills to promote residents’ physical and psychosocial well-being and provide thoughtful care in a calmer, more personal setting. The role emphasizes supportive relationships, coordinated care, teamwork, and a commitment to senior care excellence. The position is described as a weekend option day shift.

Requirements

Hold a current and valid Licensed Practical Nurse license in Indiana.
Have proficient medication management skills.
Be able to conduct thorough assessments and accurately document changes in resident condition.
Have a strong passion for geriatric nursing and commitment to senior care excellence.
Have excellent communication and interpersonal skills.

Responsibilities

Create a comforting and engaging atmosphere for residents.
Promote teamwork within the care team to meet residents’ needs.
Collaborate with nursing and other facility teams to provide coordinated and comprehensive care.
Work with the team toward excellent resident care.

Manage medications, conduct resident assessments, and accurately document changes in resident condition.
