About The Position

As a Licensed Practical Nurse (LPN) at CarDon and Associates, you will provide high-quality patient care across shifts and assist residents with their daily needs, comfort, and well-being. You will collaborate with a multidisciplinary team to develop and implement individualized care plans, monitor residents’ health, administer medications, and educate residents and their families. The role supports residents’ quality of life and health goals. University Heights is hiring full-time and part-time LPNs for day and evening shifts in memory care.

Requirements

• Current and valid Licensed Practical Nurse (LPN) license in Indiana.
• Basic Life Support (BLS) certification.

Nice To Haves

• Experience in a long-term care or assisted living environment.
• Proficiency in electronic health record (EHR) systems.

Responsibilities

• Administer medications and treatments as prescribed by the healthcare provider.
• Monitor and record vital signs and report changes in residents’ conditions to nursing staff.
• Assist residents with daily living activities, including bathing, dressing, and grooming.
• Maintain accurate and up-to-date resident records and documentation.
• Collaborate with healthcare team members to develop and implement care plans.
• Provide education to residents and their families.
