About The Position

As a Licensed Practical Nurse (LPN) at CarDon and Associates, you will provide high-quality patient care across all shifts and assist residents with daily activities to support their comfort and well-being. You will collaborate with a multidisciplinary healthcare team to develop and implement individualized care plans, monitor residents’ health, administer medications, and educate residents and their families. Your work will contribute to residents’ quality of life and health goals.

Requirements

• Current, valid Licensed Practical Nurse (LPN) license in the state of Indiana.
• Basic Life Support (BLS) certification.

Nice To Haves

• Experience in a long-term care or assisted living environment.
• Proficiency in electronic health record (EHR) systems.

Responsibilities

• Administer medications and treatments as prescribed by the healthcare provider.
• Monitor and record vital signs, and report changes in residents’ conditions to nursing staff.
• Assist residents with daily living activities, including bathing, dressing, and grooming.
• Maintain accurate, up-to-date resident records and documentation.
• Collaborate with healthcare team members to develop and implement care plans.
• Provide education to residents and their families.
