About The Position

BAYADA Home Health Care’s Phoenix Skilled Office is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) for one-to-one private duty nursing with pediatric or geriatric clients. Nurses provide individualized care in a client’s home, typically working overnight shifts of 8–12 hours. The role offers flexible scheduling and the opportunity to build relationships with clients while supporting their care and milestones. The nurse travels to one client’s home on a chosen schedule, follows and documents doctors’ orders, performs assessments, monitors the client’s condition, and records observations, interventions, and evaluations. The role provides autonomy to perform nursing skills in the home.

Requirements

Graduation from an accredited and approved nursing program, as indicated by a school transcript or diploma.
A current nursing license in good standing in the state of Arizona.
One year of experience working as a Licensed Practical Nurse.

Nice To Haves

Trach experience is preferred but not required.

Responsibilities

Travel to one client’s home according to the selected schedule.
Follow up on, execute, and properly document doctors’ orders.
Perform assessments and monitor clients’ conditions.
Document observations, interventions, and evaluations.
