About The Position

BAYADA Home Health Care’s Phoenix Skilled Office is seeking a compassionate, dedicated Licensed Practical Nurse (LPN) for one-on-one private duty home care with pediatric or geriatric clients. Nurses provide individualized care in a client’s home, with traditional shifts of 8–12 hours and a schedule nurses can choose. The role offers autonomy, hands-on care with one client per day, and the opportunity to build relationships and observe clients’ milestones. Overnight shifts are available, with starting pay of $28–$30 per hour depending on the client.

Applicants must graduate from an accredited and approved nursing program and hold a current Arizona nursing license in good standing. One year of experience as an LPN is required. Trach experience is preferred but not required.

• A current nursing license in good standing in the state of Arizona.
• One year of experience working as a Licensed Practical Nurse.

Responsibilities

• Travel to one client’s home near you according to a chosen schedule.
• Follow up on, execute, and properly document doctors’ orders.
• Perform assessments and monitor clients’ conditions.
• Document observations, interventions, and evaluations.
• Perform nursing skills autonomously in the home.
