About The Position

The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The position includes a $10,000 sign-on bonus for external candidates. Requirements
- At least 1 year of LPN or LVN experience.
- Current LPN license specific to the state or states assigned.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
Responsibilities
- Verify the plan of care with the RN before each visit.
- Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient's condition changes.
- Teach patients and caregivers self-care techniques.
- Check vital signs.
- Review medications and perform medication reconciliation at each visit.
- Perform physician-ordered specialized procedures, such as Foley catheter changes.
- Prepare equipment and materials for treatments using aseptic technique.
- Complete documentation in a timely and accurate manner.
