About The Position
The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role includes collaborating with physicians and RNs to meet patient needs, educating patients and caregivers, monitoring health, managing medications, performing ordered procedures, and documenting care. The position uses a combination of per-visit-point compensation for visits and hourly pay for certain non-visit activities. The stated annual total cash target assumes full-time employment at 40 hours per week and full productivity. A $10,000 sign-on bonus is offered to external candidates.
Requirements
- At least 1 year of LPN or LVN experience.
- Current LPN license specific to the state(s) assigned for work.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and liability insurance.
- Pass a drug test before beginning employment.
Responsibilities
- Verify the plan of care with the RN before each visit.
- Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient’s condition changes.
- Teach patients and caregivers self-care techniques.
- Check vital signs.
- Review all medications and perform medication reconciliation at each visit.
- Perform specialized procedures ordered by the physician, such as Foley catheter changes.
- Prepare equipment and materials for treatments using aseptic technique.
- Complete documentation accurately and on time.
