About The Position
The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role includes coordinating with the RN and physician to address patient needs, educating patients and caregivers, monitoring health, managing medications, performing ordered procedures, and documenting care. The position offers development opportunities and includes compensation through per-visit-point pay for visits and hourly pay for certain non-visit activities.
Requirements
- At least 1 year of LPN or LVN experience.
- Current LPN license specific to the state or states assigned.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
- Pass a drug test before beginning employment.
Responsibilities
- Verify the plan of care with the RN prior to each visit.
- Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient’s condition changes.
- Teach patients and caregivers self-care techniques.
- Check vital signs.
- Review all medications and perform medication reconciliation at each visit.
- Perform specialized procedures ordered by the physician, such as Foley catheter changes.
- Prepare equipment and materials for treatments using aseptic technique.
- Complete documentation timely and accurately.
