About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. This full-time, day-schedule role serves patients in Kenton County. The position offers a $10,000 sign-on bonus for external candidates. Compensation includes per-visit-point pay for patient visits and hourly pay for certain non-visit activities, such as orientation. The stated annual total cash target assumes full-time employment at 40 hours per week and full productivity. Requirements
- Current LPN license specific to the state or states where assigned to work.
- Valid driver's license, reliable transportation, and liability insurance.
- Current CPR certification.
- At least 1 year of LPN or LVN experience.
Responsibilities
- Verify the plan of care with the RN before each visit.
- Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient's condition changes.
- Teach patients and caregivers self-care techniques.
- Check vital signs.
- Review medications and perform medication reconciliation at each visit.
- Perform specialized procedures ordered by a physician, such as Foley catheter changes.
- Prepare equipment and materials for treatments using aseptic technique.
- Complete documentation accurately and on time.
