About The Position

This is a temporary travel assignment for a Licensed Practical Nurse (LPN) in Long Term Care at a Skilled Nursing Facility in Geneva, NY. The contract duration is 24 weeks, starting on 09/15/2026, with a day shift. The role involves providing direct and indirect skilled nursing care to adult and geriatric residents under the supervision of a Registered Nurse (RN). Responsibilities include collecting resident data, contributing to care plans, recommending revisions, and maintaining current knowledge and clinical competency. The position requires demonstrating leadership, collaborating with physicians and staff, and providing care aligned with geriatric patient needs.

Requirements

Active New York LPN licensure
Basic Life Support (BLS) certification (AHA or RQI)
Minimum 1 year of LPN experience in a long-term care setting
Ability to provide care to adult and geriatric residents
Ability to work the listed shift (days) and function effectively under RN supervision in a skilled nursing environment

Responsibilities

Provide direct and indirect skilled nursing care for assigned adult and geriatric residents under the supervision of an RN.
Collect data on residents’ physical and psychosocial status and communicate findings to the RN and the interdisciplinary team.
Contribute to the development of an age-appropriate, culturally relevant plan of care, involving the resident, family, and healthcare team, and update as outcomes are evaluated.
Recommend revisions to the plan of care as needed and coordinate referrals to ensure comprehensive resident needs are addressed.
Maintain current knowledge and clinical competency; participate in mentoring and inservice activities; uphold safety and quality standards in all patient care.
Demonstrate leadership in the professional practice setting, collaborate with physicians and other staff, and provide care aligned with geriatric patient needs.
