On-Call Licensed Practical Nurse
About The Position
Concordia Hospice of Washington is seeking an On-Call Licensed Practical Nurse (LPN) to provide comfort-focused care and support to patients and families outside regular business hours. The position requires travel throughout the South Hills region and Washington County and is suited to a nurse who can work independently and deliver meaningful, patient-centered care. The nurse responds to patient needs during evenings, nights, and weekends; provides skilled nursing focused on comfort and symptom management; assesses patient conditions and communicates updates to the care team; and offers education, reassurance, and guidance to patients and families. Care should be delivered with compassion, professionalism, sensitivity, empathy, and respect. Concordia Hospice of Washington is part of an organization formed in 2016 through a partnership between Concordia Community Support Services and Washington Health System. Its mission is to serve the aging community with a continuum of high-quality caregiving options in a Christian environment and to serve people with limited funds to the best of its ability.
Requirements
- Hold a current, active Licensed Practical Nurse license.
- Be calm, confident, and comfortable working autonomously.
- Be compassionate and attentive to patient and family needs.
- Communicate clearly and thoughtfully.
- Be organized, dependable, and responsive.
- Be comfortable traveling within assigned territories.
