About The Position

Madrona Hospice Care Inc. is seeking an On-Call Licensed Practical/Vocational Nurse to provide home hospice care. The nurse will help patients remain safe and receive care in their own environment, supporting patients and families through their transition. The role emphasizes teamwork, empathy, and high-quality, patient- and family-centered service. This position is per diem, with a rate of $50–$60 per visit.

Requirements

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• At least 3 years of nursing experience.
• Current CPR certification from the AHA.
• Valid driver’s license, reliable insured vehicle, and ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Nice To Haves

• Home health or medical-surgical background preferred.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team, including OT, PT, HHA, and other disciplines, and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely and adhere to aseptic technique.
• Perform other duties as required for patient services.
