On- Call Licensed Practical/ Vocational Nurse

About The Position
As a Licensed Practical/Vocational Nurse with Affinity Hospice Care, you will provide patient care in patients’ homes as part of home hospice care. The role focuses on helping patients stay safe and thrive in their own environment, while supporting patients and families with empathy and high-quality service. It is a per diem position paying $50–$60 per visit, with the possibility of converting to full-time based on performance if interested.
Requirements
- Graduate of an accredited practical or vocational nursing program.
- Active LPN or LVN license in California.
- Three or more years of nursing experience.
- Current CPR certification with AHA.
- Valid driver’s license, reliable insured vehicle, and ability to travel to patient homes.
- Strong communication skills and a collaborative, patient-first mindset.
Nice To Haves
- Home health or medical/surgical background preferred.
Responsibilities
- Provide direct patient care in accordance with the state Nurse Practice Act.
- Implement and follow individualized care plans established by the RN.
- Educate patients and their families on self-care techniques and diagnosis-related information.
- Document care accurately and in a timely manner.
- Collaborate with the care team, including OT, PT, HHA, and other disciplines, and communicate patient needs to the RN or Clinical Supervisor.
- Use supplies and equipment safely, adhering to aseptic technique.
- Perform other duties as required for patient services.
