About The Position

Madrona Hospice Care Inc. is seeking an On-Call Licensed Practical/Vocational Nurse to provide home hospice care. The nurse will help patients stay safe and receive care in their own environment, working in a supportive, family-oriented setting focused on teamwork, empathy, and high-quality service. This role is per diem, with compensation of $50–$60 per visit. The posting states that the position may be converted to full-time based on performance if desired.

Requirements

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• At least 3 years of nursing experience.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Nice To Haves

• Home health or med/surg background preferred.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team, including OT, PT, HHA, and other disciplines, and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
