About The Position

Concordia Hospice of Washington is seeking a compassionate and dependable On-Call Licensed Practical Nurse (LPN) to provide comfort-focused care and support to patients and families outside regular business hours. The role is suited to a nurse who can work independently and deliver meaningful, patient-centered hospice care. It requires travel throughout the South Hills region and Washington County.

The LPN responds to patient needs during evenings, nights, and weekends; provides skilled nursing care focused on comfort and symptom management; assesses patient conditions and communicates updates to the care team; and offers education, reassurance, and guidance to patients and families. The nurse is expected to deliver care with compassion, professionalism, and sensitivity and to represent Concordia Hospice of Washington with empathy and respect.

Concordia Hospice of Washington is a partnership between Concordia Community Support Services and Washington Health System. It serves patients in Washington and Greene counties, providing home care and inpatient hospice care focused on comfort, peace, and dignity for people with incurable illnesses and their families.

Requirements

• Current, active Licensed Practical Nurse license.
• Calm, confident, and comfortable working autonomously.
• Compassionate and attentive to patient and family needs.
• Clear and thoughtful communication skills.
• Organized, dependable, and responsive.
• Comfortable traveling within assigned territories.
