On- Call Licensed Practical/ Vocational Nurse

About The Position

As a Licensed Practical/Vocational Nurse with Affinity Hospice Care, you will provide direct care to patients in their homes and help them stay safe and comfortable. You will implement care plans established by an RN, educate patients and families, document care, and collaborate with the care team. The role is per diem, with a rate of $50–$60 per visit. The schedule is flexible, and the position may convert to full-time based on performance if desired.

Requirements

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• Three or more years of nursing experience.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Nice To Haves

• Home health or med/surg background preferred.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team, including OT, PT, HHA, and other disciplines, and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
