About The Position
Crown Heights Center for Nursing and Rehabilitation is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join its skilled nursing team in Brooklyn, New York. The LPN provides high-quality care to residents, supports their medical and emotional needs, and promotes comfort and well-being. The role works closely with the healthcare team to support residents’ health, healing, and rehabilitation. The Allure Group operates six rehabilitation centers across Brooklyn and Manhattan, specializing in post-acute and sub-acute skilled nursing and rehabilitation. Requirements
- Valid New York State LPN license.
- Strong communication, customer service, and interpersonal skills.
- COVID-19 vaccination required.

Responsibilities
- Conduct rounds to observe residents and assess nursing care.
- Review and implement Nursing Care Plans addressing residents’ physical, emotional, and rehabilitative needs.
- Administer and transcribe physician orders accurately.
- Collaborate with staff to deliver comprehensive, high-quality care.
- Manage medication ordering, administration, and documentation.
- Maintain accurate and thorough clinical records.
- Participate in interdisciplinary meetings to improve care and resolve issues.
- Provide direct patient care as needed, including monitoring vital signs and administering treatments.
- Assist with identifying and addressing care improvement opportunities.
- Perform additional duties within the LPN scope of practice as necessary.
