About The Position

Harlem Center for Nursing and Rehabilitation is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) for its skilled nursing team in Harlem, New York. The LPN provides high-quality care to residents, supports their medical and emotional needs, and promotes their comfort and well-being. The role works closely with the healthcare team to support health, healing, and rehabilitation.

Requirements

• Valid New York State LPN license.
• Strong communication, customer service, and interpersonal skills.
• COVID-19 vaccination required.

Responsibilities

• Conduct rounds to observe residents and assess nursing care.
• Review and implement Nursing Care Plans addressing residents’ physical, emotional, and rehabilitative needs.
• Accurately administer and transcribe physician orders.
• Collaborate with staff to deliver comprehensive, high-quality care.
• Manage medication ordering, administration, and documentation.
• Maintain accurate and thorough clinical records.
• Participate in interdisciplinary meetings to improve care and resolve issues.
• Provide direct patient care as needed, including monitoring vital signs and administering treatments.
• Help identify and address opportunities to improve care.
• Perform additional duties within the LPN scope of practice as necessary.
