Licensed Practical Nurse, Home Visits
About The Position
As a CenterWell Home Health nurse, the Licensed Practical Nurse reports to the Clinical Manager and provides personalized, one-on-one skilled nursing care to patients in their homes under the direction of a Registered Nurse. The role supports patients in regaining independence and recovering in the comfort of their homes. The nurse implements individualized care plans in collaboration with patients, families, and the healthcare team; educates patients and families about disease management, treatment options, and self-care; and maintains accurate records of visits, interventions, and outcomes. The role also requires timely communication with patients, physicians, the Clinical Manager, and other healthcare team members regarding patient or family needs and changes in status, as well as participation in care coordination and discharge planning.
Requirements
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- One year of experience as an LPN/LVN in a clinical setting.
Nice To Haves
- Nursing experience in a Home Health or Hospice setting.
- HCHB experience.
Responsibilities
