About The Position
BAYADA Home Health has an immediate opening for a part-time Licensed Practical Nurse to perform home health visits through its Dartmouth office. The office serves adult and geriatric clients in Taunton, Raynham, and surrounding towns. The role offers flexible day schedule options and work/life balance. The nurse provides compassionate home health care that supports clients' comfort, independence, dignity, and ability to remain safely at home.
Requirements
- Hold a current Licensed Practical Nurse license in Massachusetts.
- Have a minimum of one year of recent LPN experience.
- Be able to travel to assigned cases.
Nice To Haves
- Prior home care experience is preferred but not required.
Responsibilities
- Make home visits to clients living in designated territories.
- Perform assigned duties, including administering medication and providing wound care, treatments, and procedures.
- Monitor clients' conditions and report changes to the Clinical Manager and Client Services Manager.
- Follow up on, execute, and properly document doctors' orders.
- Perform client assessments as necessary.
- Provide case management and coordination.
- Accurately document observations, interventions, and evaluations related to client care and services using electronic medical records on a touch pad tablet.
