About The Position
BAYADA Home Health has an immediate opening for a part-time Licensed Practical Nurse to perform home health visits for its Dartmouth office. The office provides home health services to adult and geriatric clients in Wareham, Marion, Mattapoisett, and surrounding towns. The role involves visiting clients in designated territories and providing assigned nursing care. BAYADA’s purpose is to help people of all ages live safely at home with comfort, independence, and dignity, delivering care with compassion, excellence, and reliability.
Requirements
- Current Massachusetts license as a Licensed Practical Nurse.
- At least one year of recent experience as an LPN.
- Ability to travel to assigned cases.
Nice To Haves
- Prior home care experience is preferred, but not required.
Responsibilities
- Make home visits to clients living in designated territories.
- Perform assigned duties, including medication administration, wound care, treatments, and procedures.
- Monitor clients’ conditions and report changes to the Clinical Manager and Client Services Manager.
- Follow up on, execute, and properly document doctors’ orders.
- Perform client assessments as necessary.
- Provide case management and coordination.
- Accurately document observations, interventions, and evaluations related to client care management and services provided using electronic medical records on a touch pad tablet.
