About The Position

BAYADA Home Health Care is seeking a Licensed Practical Nurse (LPN) to join its Yavapai Visits team in Prescott. The LPN provides home health care to clients in accordance with established client care plans and physicians’ treatment plans. The role includes performing and documenting nursing care and treatments, reporting changes in client condition, and carrying out new physician orders. BAYADA provides paid training from day one, ongoing clinical support, and a supportive team environment.

Requirements

• Current Arizona nursing license in good standing.
• Graduation from an accredited and approved nursing program, as indicated by a school transcript or diploma.
• One year of clinical experience as a Licensed Practical Nurse.

Responsibilities

• Execute established client care plans and physicians’ plans of treatment.
• Provide patient care and treatments, including taking blood pressure and other vital signs, wound care, collecting blood samples, and administering prescribed medication in accordance with the provider’s treatment plan.
• Report changes in clients’ conditions to the nursing supervisor and physician.
• Execute and properly document new physician orders.
• Accurately document observations and nursing interventions using a touch-pad tablet.
