About The Position

BAYADA Home Health Care is seeking a Licensed Practical Nurse (LPN) to join its Yavapai Visits team in Prescott. The role delivers home health care to clients and their families with compassion, excellence, and reliability. The position is listed as up to full-time.

The LPN carries out established client care plans and physicians’ plans of treatment, provides and documents nursing care, and reports changes in client condition. BAYADA provides paid training in the office, in the home, and online, as well as around-the-clock clinical support.

Requirements

• Hold a current nursing license in good standing in Arizona.
• Graduate from an accredited and approved nursing program, as indicated by a school transcript or diploma.
• Have one year of clinical experience as a Licensed Practical Nurse.

Responsibilities

• Execute established client care plans and physicians’ plans of treatment.
• Provide patient care and treatment, including taking blood pressure readings, performing wound care, measuring vital signs, collecting blood samples, and administering prescribed medication in accordance with the provider’s treatment plan.
• Report changes in clients’ conditions to the nursing supervisor and physician.
• Execute and properly document new physician’s orders.
• Accurately document observations and interventions related to nursing care using a touch pad tablet.
