About The Position

Elara Caring is hiring a Licensed Practical Nurse to provide behavioral health visits in patients’ homes in the Bridgeport, Stratford, and Trumbull areas. The role contributes to coordinated, compassionate care by following individualized care plans and physician orders, monitoring patient progress, educating patients and caregivers, and working with the RN Case Manager and other members of the care team. The position is PRN and is described as an opportunity to earn extra money.

Requirements

• Current state license as a Licensed Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• One year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.

• Experience in a hospice or home health environment.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.
