About The Position

Community Care, Inc. is seeking a compassionate and dedicated Licensed Practical Nurse to provide one-on-one pediatric private duty nursing care for a 16-year-old male child in a supportive home setting in Scottdale, Pennsylvania. The role focuses on personalized, family-centered care and building a meaningful connection with the child and family.

The position is full-time, Monday through Friday, from 10:00 PM to 6:00 AM. Weekend shifts are also available. The nurse will provide care involving a tracheostomy and ventilator, along with other clinical and daily-living support.

Requirements

• Current LPN or RN license in Pennsylvania.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience.
• CPR certification.
• Reliable transportation and a valid driver's license.
• A caring nature, strong independence, and a passion for pediatric care.

Nice To Haves

• Home health experience is preferred, but not required.

Responsibilities

• Assist with activities of daily living (ADLs).
• Provide G-tube care and feedings, including aspiration precautions.
• Monitor vital signs and seizures.
• Provide and monitor tracheostomy and ventilator care.
• Administer oxygen.
• Administer medications safely and accurately.
• Communicate clearly and compassionately with the care team and family.
• Deliver exceptional, family-centered pediatric care.
