About The Position
This is a 12-week travel assignment for a Licensed Practical Nurse (LPN) in a long-term care setting, specifically a Skilled Nursing Facility / LTC - Nursing Home in Laconia, New Hampshire. The position offers guaranteed hours and a night shift schedule.
Requirements
- Active Licensed Practical Nurse license in New Hampshire
- Ability to work night shift (3x12-hour nights)
- Nursing diploma or equivalent from an approved LPN program
- Experience in a long-term care or skilled nursing setting preferred
- Strong communication and teamwork skills; ability to follow care plans and facility policies
Responsibilities
- Administer prescribed medications and document administration per policy
- Conduct routine nursing assessments, monitor vital signs, and identify changes in resident condition
- Implement and update care plans under RN supervision; assist with activities of daily living and mobility
- Ensure accurate charting and documentation in the electronic health record; manage medication administration records
- Maintain safety, infection control, and resident welfare; respond to emergencies per established protocols
- Collaborate with the interdisciplinary team to ensure quality care and timely communication with residents’ families as appropriate
