About The Position

Pinnacle Home Care is hiring a PRN Licensed Practical Nurse (LPN) to provide compassionate, patient-centered skilled nursing care in patients’ homes. The nurse works under RN supervision and carries out care plans while supporting positive patient outcomes, accurate documentation, and regulatory compliance. The role includes coordinating visits with other staff and following agency policies and quality, complaint-management, patient-rights, and discharge processes.

Requirements

• Current state-specific LPN or LVN license.
• Graduate of an accredited nursing program.
• Valid BLS certification.
• At least one year of nursing experience.
• Excellent communication, organizational, and time-management skills.
• Valid driver’s license and access to a reliable, insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
• Ability to walk, stand, stoop, and occasionally lift or turn patients.

Nice To Haves

• Home health or medical/surgical nursing experience is preferred.

Responsibilities

• Administer skilled nursing care under RN supervision.
• Perform curative, rehabilitative, or preventive care tasks as outlined in patient care plans.
• Use clinical expertise and nursing education to make informed decisions that support patient care and positive outcomes.
• Complete visit reports and required documentation promptly and accurately in the electronic medical record system.
• Confirm daily visit schedules to coordinate care with other staff.
• Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
• Follow the agency’s process for managing and addressing customer complaints.
• Uphold patient rights in accordance with agency service agreements and admission policies.
• Understand and follow agency policies regarding appropriate patient discharge and reasons for discharge.
